Case Planner Position Tiers:
Case Planner Tier 1 -- $50,000 per year, MSW degree, Working towards LMSW, recent MSW grad, Relevant human service / social service and school experience, and field placement experience (2000 hours of clinical practicum) OR minimum 5 years of case planning experience in a clinical setting (psychological functioning)
Case Planner Tier 2 - - $55,000 per year, MSW degree, LMSW License, 2 year post-grad experience, relevant human service / social service and school experience, and field placement experience (2000 hours of clinical practicum), 2 years of post-grad experience, Licensed with entry-level experience, Must supervise MSW and relevant interns as a Task Supervisor
Case Planner Tier 3 -- $61,000 per year, MSW degree, LMSW License, 3.5 years of post-grad experience, Relevant human service / social service, social work and school social work and counseling experience, 3.5 years of post-grad experience, Licensed with 3+ years of experience, Must supervise MSW and relevant interns as a Site Supervisor
The salary for the finalist selected for this role will be determined based on various factors, including Departmental or Program budgets, the candidate’s qualifications, experience, education, licenses, specialty, and training. About I’RAISE: I'RAISE Girls & Boys International Corporation, founded in the heart of the North Bronx in 2012, began as a mentoring program for at-risk female youth living in New York City Authority Housing. Under the visionary leadership of CEO Shanequa Moore, the organization quickly expanded its reach and impact. Responsibilities:
Program Participation:
I'RAISE Integrative Mental Wellness (IMW) Program within our School Department.
Manage ACS Mandated Cases (preventive services) and non-ACS Mandated Cases (clinical mental health services under the IMW Program).
Case Management and Service Delivery:
Provide prevention services to high-risk families to address mental health and social service needs.
Conduct comprehensive assessments of family strengths, needs, and goals.
Develop individualized service plans in collaboration with families, incorporating their input and preferences.
Provide ongoing case management support, including crisis intervention, advocacy, and referrals to community resources.
Coordinate with school staff, community partners, and other service providers to ensure seamless service delivery.
Monitor progress toward goals, making adjustments to service plans as needed to ensure positive outcomes.
Maintain accurate and up-to-date case records in accordance with agency policies, regulatory requirements, and best practices.
Participate in team meetings, case conferences, and required training opportunities to promote continuous professional development.
Provide guidance and mentorship to less experienced staff as needed.
Collaboration and Program Improvement:
Collaborate with program leadership to assess program effectiveness and identify areas for improvement to enhance service delivery.
Maintain manageable caseloads to ensure the quality of case management services, allowing for meaningful family engagement and professional growth opportunities for staff.
Student and Family Support:
Review student data and make appropriate referrals for mental health services.
Conduct comprehensive intake assessments with students and families.
Facilitate ongoing family counseling sessions, ensuring a holistic approach to treatment.
Develop individualized and family-centered treatment plans, working closely with families to achieve shared goals.
Engage parents and guardians in treatment planning and progress monitoring, ensuring their active participation in the process.
Develop discharge plans and conduct ongoing assessments to track student and family progress and ensure continued support.
School Collaboration:
Build strong, collaborative relationships with school personnel to ensure the effectiveness of the IMW Program in meeting student needs.
Provide consultation, brief interventions, and behavioral planning support to teachers and administrators.
Collaborate with school officials (principals, teachers) to address student needs and ensure integrated service delivery.
Educate parents and school staff regarding available resources and programs for students with mental health and behavioral challenges.
Clinical Expertise:
Conduct family screenings and develop comprehensive treatment plans that reflect the unique needs of each family.
Facilitate on-site conflict resolution and peer mediation sessions for students to foster a positive school climate.
Conduct individual and group counseling sessions, ensuring that therapeutic interventions are aligned with evidence-based practices.
Maintain detailed progress notes and documentation to track student improvement and ensure compliance with regulatory requirements.
Compliance and Documentation:
Manage case files and complete required forms and reports in accordance with district, state, and federal regulations.
Maintain accurate and up-to-date records in line with agency policies and regulatory requirements.
Complete any other related duties/tasks as assigned by supervisor to support program goals and operations.
Minimum Qualifications
Master’s degree in Social Work (MSW), Counseling, Psychology, or a related field.
Active New York State LMSW or LMHC License required
Minimum of 3.5 years of post-grad experience in case management, social work, family support, clinical mental health services, particularly with high-risk families.
Experience working with school systems and knowledge of educational resources and support services for students.
Experience conducting individual and group counseling sessions with students and families.
Excellent communication, interpersonal, and organizational skills
Proficiency in Google Suite and case management software preferred
Ability to travel to NYC public schools as needed.
Excellence in project management, teamwork, analytical thinking, and client/customer awareness.
NYC resident with a valid Social Security Number (no relocation assistance or visa sponsorship provided) for background check/clearance requirements
Must be able to commute to any borough in New York City
